6 Months Old Neglected Monteggia Lesion in a Blind Woman - Surgical Treatment

Description

This presentation by Dr. Sergio Rowinski, from Sao Paulo, Brazil, discusses a complex medical case involving a 64-year-old woman with a neglected Monteggia lesion in her right elbow. The patient, who is diabetic and blind, suffered a fall in September 2009, leading to significant deformity and limitation in her arm, which went untreated for several months. Dr. Rowinski details the clinical examination, revealing severe pain and deformity, along with x-ray results showing nonunion in the proximal ulna and disappearance of the radial head. After surgery in April 2010, an extended Boyd's approach was utilized, where the ulna was repaired with a locked plate, and a radial head prosthesis was installed. Post-operative progress showed improvements in range of motion and pain relief, with follow-up x-rays exhibiting healing of the elbow. By six months post-surgery, the patient regained functional use of her arm, allowing her to perform daily activities. The presentation emphasizes the complexity of Monteggia lesions, the importance of proper diagnosis, and successful surgical intervention.
