All Suture Technique for Anterior Glenoid Bone Grafting
Description
The video demonstrates a surgical procedure involving iliac crest grafting for the anterior glenoid using an all-suture technique. The patient is positioned in the beach chair position, and the surgeon employs a deltopectoral approach with a subscapularis split to access the anterior glenoid, exposing the neck for assessment of bone loss or malunion. A saw or burr is utilized to remove remnants and prepare the glenoid surface for grafting.
Guided by a green cannula obturator, a FibreTak drill is used to create holes for two anchors in the glenoid neck, from which sutures are extracted and organized into pairs with fiber wires and suture tapes. The sutures are shuttled through the graft and paired before being secured to a tibial plate, ultimately parachuting the graft onto the glenoid neck.
The process involves sequentially tying the sutures to establish firm stability, beginning with fiber wire sutures before progressing to non-sliding knots with suture tapes. Post-stabilization, any excess graft material is trimmed for optimal fit against the glenoid. The anterior capsule is repaired using 1.6 mm anchors and sutures placed in a mattress fashion to complete the procedure.
The result exposes a stable graft on the glenoid, with successful capsule repair noted, concluding the surgical operation.
