Wrist Fusion With Proximal Row Carpectomy
Description
In this educational video, Dr. Bijayendra Singh presents a detailed case study of a 60-year-old female patient who underwent a wrist fusion with distal radio-ulnar reconstruction. The patient originally sustained a seemingly simple distal radius fracture, which was conservatively managed but later led to significant functional issues due to a deformity and shortening of the radius. After six months of therapy, the decision was made to proceed with surgical fusion to correct the functional disadvantage and address the deformity.
The video progresses through the surgical procedure, illustrating the incision and methodical approach to gaining access to the wrist joint. Key steps include careful dissection of the skin and underlying structures, the removal of damaged bone fragments, and the preparation of bone surfaces for fusion.
Dr. Singh meticulously details the surgical technique, emphasizing the importance of maintaining hemostasis, the careful release of wrist ligaments, and the use of bone grafts from excised carpal bones to enhance the fusion site. Various tools and techniques are shown, including stay sutures for retraction, the use of high-speed burrs for decortication, and ensuring proper alignment of the wrist during plating.
Post-operative care is briefly discussed, including rehabilitation and follow-up radiographs which reflect successful alignment and evidence of fusion at the wrist joint. The video concludes with an invitation to visit the channel for more surgical educational content.
