This expert insight addresses the usage of screw fixation, noting that regardless of whether screws are employed, long-term outcomes remain consistent if a good press fit is achieved in the acetabular component.
Dr Vigdorchik emphasizes the need to divide the acetabulum into four quadrants for safe screw placement, highlighting the significance of knowing which areas carry a higher risk of vascular injury. Surgeons are advised to drill screws primarily in the posterior superior quadrant, deemed the safest, while being wary of the anterior superior quadrant, which poses a much higher risk of damaging critical vascular structures. The protocol encourages surgeons to keep their finger in the sciatic notch for orientation during the procedure, ensuring accurate placement of screws relative to the anatomical landmarks, thereby minimizing potential complications.
