Cervical Disk Arthroplasty

Description

The video presentation by Sheeraz Qureshi focuses on the surgical technique of cervical disk arthroplasty, highlighting its growing role as an alternative to the traditional anterior cervical discectomy and fusion, which remains the gold standard for cervical disk pathology treatment. The case study features a 43-year-old female patient suffering from left-sided neck and shoulder pain, along with diminished sensation and strength in her left hand, induced by a large left-sided C5/6 disk herniation confirmed via MRI. After exhausting conservative management options without relief, the patient is presented with surgical solutions, including the option of arthroplasty.



The surgical procedure begins with a well-prepared operating room setup utilizing a Jackson table and draped C-arm for imaging. The technique involves careful dissection and retraction of muscle for optimal visualization, followed by the placement of Caspar pins for stabilization and alignment during the disk removal.



In addition to the meticulous removal of disk material using various cutting and rongeur instruments, the surgical team ensures thorough decompression of the dural sac and foramen, preparing the endplates for implant placement. Following the removal of herniated material and osteophyte adjustment, implant trialing is emphasized to achieve optimal coverage while ensuring the implant fits correctly within the disk space.



The use of a Kleiner implant is showcased, demonstrating additional stability during the procedure. Intraoperative fluoroscopy assists in confirming the accurate placement of the implant before final adjustments and closure of the surgical site, emphasizing the importance of meticulous hemostasis and wound management post-surgery.



A follow-up radiograph two weeks post-operation reveals the successful fit of the implant, concluding the presentation with thanks for the audience's attention.
