In this presentation, Catherine Hui discusses techniques for ensuring accurate and safe femoral tunnel placement during knee surgeries, particularly focusing on the importance of using a loose anchor and larger grafts to reduce the risk of graft rupture. Hui highlights the risks associated with iatrogenic injury to the medial femoral condyle when using femoral reamers, emphasizing the need for proper equipment and direct visualization during the reaming process.



She details her method of direct observation when inserting the reamer, evaluating if it fits properly before proceeding. If adjustments are necessary, she outlines a two-step approach: firstly, using a slotted cannula technique if the reamer is close to fitting, to protect the medial condyle; or alternatively, employing a modified drilling technique if the reamer is significantly oversized, which includes flexing the knee and guiding a pin through the reamer into the tunnel. Hui showcases these techniques with cautionary advice regarding the careful removal of instruments to avoid complications, concluding with a demonstration of the procedures.
