Improving Pathways for Frail and/or Multimorbid Patients

Description

In this presentation led by Dr. Scarlett McNally, Deputy Director of the Center for Perioperative Care at Guy's and St Thomas', the challenges of managing surgical patients, particularly the elderly, are discussed. McNally highlights the increasing complexity of surgical patients driven by aging, which often leads to more degenerative diseases and heightened risks of complications and adverse outcomes.

She emphasizes the importance of a geriatric approach to surgical care, acknowledging that aging associates with issues like frailty and diminished functional reserve. Citing a 2012 paper from The Lancet, McNally notes the rapid increase in co-morbidities with age, stressing the necessity for comprehensive assessment to cater to the unique needs of older surgical patients.

McNally introduces the POPS (Perioperative Care for the Older Person) program, which employs a proactive model focusing on both elective and emergency surgical patients, emphasizing the need for thorough medication reviews and lifestyle recommendations. The talk advocates for shared decision-making within surgical teams and urges practitioners to enhance their communication skills to ensure patients are adequately informed and empowered in their care choices.

With statistics showcasing improved patient outcomes following comprehensive assessments, she strongly recommends incorporating geriatric expertise in surgical settings to enhance patient safety and overall care quality. The session also calls for a cultural shift in the treatment of older patients, encouraging active engagement and improvement strategies tailored to their specific needs.
