He highlights that approximately 33% of patients with chronic lateral ankle instability also have co-existing osteochondral lesions, primarily located on the medial talar dome. Mr Chlebinskas emphasizes arthroscopic techniques for unstable cartilage removal combined with bone marrow stimulation methods like microfracturing or drilling, showing a high success rate of 85% to 100% in restoring functionality and encouraging return to physical activity. He also recommends that ligament repair should be conducted concurrently to reduce the likelihood of recurrence and long-term degeneration.
