PJI Setting The Scene

Description

The presentation by Dominic Meek discusses the pressing issue of periprosthetic joint infections (PJI) and the introduction of the new BOAST guidelines. Starting with an overview, Meek highlights the stagnation in infection rates, noting they hover around 0.9% despite an increase in surgical volume, indicating a rising challenge in managing these infections. He emphasizes that while the UK's efforts in addressing PJI have gained attention, the complexity of the pathogens involved—particularly their ability to form biofilms and communicate through quorum sensing—poses significant hurdles.

Meek explains that although the incidence rate may seem low, the implications of PJI are severe, especially as patients are followed over time, with infection rates doubling by the fifth year. The dire outcomes associated with infected implants, including poor survival rates and increased mortality, underscore the critical nature of infections post-surgery, particularly in older patients. Meek advocates for vigilant management strategies, including the formation of multidisciplinary teams (MDTs) to enhance surgical outcomes and address the increased demand for hip replacements exacerbated by post-pandemic recovery efforts.

The presentation culminates in an introduction of the panel, comprising seasoned professionals who will delve deeper into the management and treatment of PJIs. The session aims to foster interaction, case discussions, and a collaborative approach to refining existing protocols, ultimately improving patient care in orthopedic practice.
