Periprosthetic Fractures – What We’ve Learnt So Far, What We Need From You

Description

In this presentation, Jonathan Evans, a lecturer from Bristol, discusses the crucial topic of periprosthetic fractures, which occur around nails, screws, plates, and primarily around hip or knee replacements. He emphasizes a significant shift in research focus from just the hardware involved in hip replacements to a more patient-centered approach, aiming to understand the implications these fractures have on patients' lives. Evans highlights the lack of current data on the incidence of these fractures in the UK, despite the existence of a plethora of research papers on the subject. He addresses the shortcomings of the National Joint Registry in capturing all periprosthetic fracture cases, particularly those that do not require revision. Through recent studies, he points out that approximately 5,000 admissions occur yearly for periprosthetic fractures, primarily around hips, yet many remain unreported.

He reviews data from his own investigations, noting that a large number of these fractures are treated conservatively rather than through surgical revision, revealing a considerable gap in our understanding of patient outcomes in these scenarios. The continuing effort to enhance data collection and reporting in this area is vital. Evans stresses that further research should not focus only on surgical techniques and specific materials but on the broader patient experience and satisfaction. Ultimately, he urges all stakeholders involved in managing these fractures to contribute to data collection efforts that will improve patient care and outcomes.
