Pivot Shift Test
Description
Dr Laurie Hiemstra gives her expert insights and discusses how the pivot shift test is a clinical examination used to assess the stability of the knee joint, particularly for diagnosing injuries to the anterior cruciate ligament (ACL). In this test, the clinician holds the leg straight, identifying a subluxation of the tibia. As the knee is flexed to about 30 degrees, the tibia is observed to shift back into its normal position, demonstrating a reduction of the subluxation. This reduction occurs as the knee is flexed, with an emphasis placed on adding axial load and mild internal rotation of the tibia to enhance the results of the test, thereby accentuating the pivot shift phenomenon.
