Posterior Labrum Needle Mattress Stitch Repair

Description

This video tutorial demonstrates the surgical procedure for posterior labral repair using a percutaneous knotless mattress suture technique. The surgery takes place on the right shoulder while the patient is positioned in lateral decubitus. Initially, a posterior labral tear is identified and assessed using a camera inserted through an anterior portal. A spinal needle is then used to plan an accessory portal for suture positioning.



The procedure involves debriding the rough edges of the labrum with a shaver, followed by the use of a fiber stick suture introduced through the labrum to retrieve it via the accessory portal. A mattress suture is created by passing the needle through the labrum while ensuring the capsule is not penetrated, establishing a loop for the second limb of the suture.



Subsequently, a pilot hole is drilled using a 2.9 mm drill, into which a bio-composite push lock anchor is inserted along the sutures. The ends of the suture are trimmed flush, and the process is reiterated with a second suture for reinforcement. Finally, both suture limbs are retrieved, and the glenoid is drilled again for additional anchor application, completing the surgical repair once the suture tails are cut flush.
