Superior Capsule Reconstruction
Description
This video details a surgical procedure for superior capsule reconstruction using the Arthrex DX matrix in a 70-year-old male patient with an unreconstructable supraspinatus tear, while preserving the intact subscapularis and infraspinatus. The patient is positioned laterally. A shaver is used to decorticate the greater tuberosity and superior glenoid. Biocomposite suture tack anchors are placed in the superior glenoid, and swivel lock anchors loaded with fiber tape are secured to the tuberosity footprint.
Measurements of anchor placements are taken and used to cut DX matrix patches that are then whip-stitched and punctured for suture passage corresponding to the anchor locations. The glenoid sutures are secured, positioning the graft over the glenoid. Medial row sutures are passed through the graft and connected using a speed bridge technique with additional fixation on the infraspinatus to ensure stability. After tying down all sutures, the repair process concludes successfully.
