The Operative Approach for Peroneus Brevis Tears
Description
Dr Donatas Chlebinskas gives his expert insights and outlines a typical surgical approach to treat a peroneus brevis tear, following the Krause and Brodsky classification. The procedure begins with exposing the perineal sheath to assess both the peroneus brevis and longus tendons. Any degenerative split segments of the peroneus brevis are excised. If over 50% of the peroneus brevis is deemed non-viable, a tenodesis procedure is performed, where the thumb of the peroneus brevis is attached end-to-side to the peroneus longus at the retromolar groove. This method aims to maintain eversion strength through the peroneus longus muscle while also addressing underlying issues like retinacular instability or a low-lying muscle belly, in order to minimize the chances of recurrence.
