Total Elbow Replacement

Description

In this instructional video, Bijayendra Singh demonstrates a total elbow replacement procedure on a 65-year-old woman suffering from left-sided osteoarthritis. The presentation begins with the patient positioned in lateral decubitus and the application of a sterile tourniquet. Singh outlines the surgical approach, emphasizing the importance of a curved skin incision that minimizes tension and enhances exposure of the ulnar nerve.

As the dissection progresses, various techniques such as a triceps peel are discussed, followed by the meticulous isolation of the ulnar nerve to prevent damage during the procedure. The video covers the detailed steps of joint dislocation, ligament removal, and radial neck osteotomy, preparing for the challenging aspects of the operation including osteophyte removal and accessing the medullary canal of the humerus.

Singh showcases the process of reaming and broaching the humerus and ulna to create a suitable environment for the prosthetics while ensuring alignment for optimal function. With careful descriptions of tools and techniques, he proceeds through trial fittings of the prosthesis, highlighting critical moments such as cement application to secure the components in place.

Final assembly and checks are demonstrated, alongside closure techniques that ensure the integrity of the extensor mechanism. Post-operative care is briefed, revealing good outcomes with radiographs confirming proper placement of the prosthesis. The video concludes with encouragement for viewer engagement and further learning through additional resources provided.
