Four-Corner Arthrodesis With Differing Methods of Osteosynthesis: A Systematic Review and Meta-Analysis

In this presentation, Emmitt Hayes discusses a systematic review and meta-analysis he conducted with colleagues Yanzhao Cheng, Dr. David Sauder, and Dr. Laura Sims, which examines outcomes following four-corner fusion, a procedure for advanced wrist arthritis, with various fixation methods. The presentation covers the methodology used for their systematic review adhering to PRISMA guidelines, the search process for relevant studies, and the criteria for inclusion and exclusion of studies. While the overall transformation union rate from their analysis was found to be 97%, the results indicated no significant differences in union rates or clinical outcomes when comparing different methods of fixation, including K-wires, screws, and various types of plates. Notably, while some differences were reported in terms of range of motion and grip strength, they were deemed unlikely to be clinically relevant. The findings suggest that surgeons can choose the fixation method they are most comfortable with, but emphasize the need for higher-quality randomized controlled trials in the future.
