Pathways to Competence: Exploring the Basis of Operative Competency Decisions in Trauma and Orthopaedic Training

Description

This presentation by orthopedic registrar Simon Fleming outlines his PhD research focused on the concept of operative competency in surgery. He explains that traditional discussions surrounding operative competency often conflate it with technical skills, without a clear definition or exploration of what it encompasses. Through qualitative interviews with trainers and trainees in the UK and Canada, Fleming identifies key components of operative competency, emphasizing the importance of insight, self-assessment, and emotional intelligence over mere technical ability. He develops a model highlighting practical skills, planning, professionalism, and interpersonal skills, but ultimately finds that insight is the defining characteristic of a competent surgeon. The presentation raises critical questions about the training and assessment of surgical competencies, especially regarding how to evaluate and nurture traits such as self-awareness and reflective practice. Fleming concludes by suggesting that the current methods of assessment may need reevaluation to better align with the insights gained from his research, promoting a more holistic approach to surgical training.
