Predictors of Radiographic Osteoarthritis at 5 Years After Anterior Cruciate Ligament Reconstruction

In this presentation, Dr. Nicholas Mohtadi discusses findings from a long-term randomized trial examining the prevalence of radiographic osteoarthritis in patients who underwent ACL reconstruction. The trial involved screening 745 patients, randomizing 330 into three different surgical techniques, and achieving substantial follow-up results. Key statistics revealed that while the overall prevalence of osteoarthritis after ACL reconstruction was 33%, this was notably lower than previous literature reports. The analysis segmented findings by knee compartment, differentiating rates of osteoarthritis as 12% in the medial compartment, 22% in the lateral compartment, and only 3% in the patellofemoral compartment, highlighting the varying predictive factors across compartments. Notably, the overall quality of life for patients remained good at the five-year mark despite these radiographic changes. Challenges included event rates impacting the robustness of predictive modeling, and the ongoing need for further longitudinal studies to refine understanding. The session concluded with acknowledgment of the complexities involved in determining the correlation between radiographic findings and clinical symptoms.
