Use of Vascularized and Non-vascularized Bone Grafting in Scaphoid Non-union: A Prospective Randomized Control Trial

Approximately 50 patients were enrolled in the study, with strict inclusion criteria, excluding those with advanced degeneration or known avascular necrosis (AVN) to maintain a homogeneous cohort. The results indicated that 95% of patients with the vascularized graft achieved union compared to 90% in the iliac crest group, although the difference was not statistically significant. Notably, smoking emerged as a significant confounder in the study's outcomes; smokers were found to be significantly less likely to achieve union across both graft types. Post-adjustment for smoking, vascularized grafts demonstrated a notable efficacy advantage, reinforcing the importance of considering lifestyle factors in surgical outcomes.
